Coordinated Entry for Homeless Services

Experiencing homelessness?

Slept in a vehicle, outside, or shelter last night?

There are programs throughout Pima County to help you. Several can be found using Coordinated Entry!

Housing services begin with a quick Housing Questionnaire with one of our Community Access Points.
Referrals are based on your unique needs and services in your area.
Accessible for people with disabilities.
Complete one assessment at any location - no need to get on multiple lists.
Coordinated Entry is a way for people experiencing homelessness, or youth at imminent risk of homelessness, to get the help they need. Housing services are accessed through a single Housing Questionnaire which is completed at any local Community Access Point. Based on the assessment, homeless individuals and families are placed on the community-wide priority list for housing assistance from which dozens of local programs accept referrals.

Coordinated Entry prioritizes people for referral to housing services based on the severity of their needs and the availability of resources. Households complete only one assessment to determine what services they may be eligible for. This research-based assessment tool helps housing specialists identify each individual or family’s unique needs and resources so they can be connected to the help they need.

Coordinated Entry provides data to help our community better plan for housing needs. All assessments are centralized so we can better track requests for assistance, referrals, services provided and contributing factors to homelessness.

What to expect when completing a Coordinated Entry assessment by phone

You may be asked to make an appointment for an assessor to give you a call back or forwarded to voicemail. If this happens, you may call another Access Point or wait for a return call. They will return your call within 1 business day.

You may be referred to a different agency or phone number on this list.

You will be read a Release of Information that normally requires a signature, but you will only be assessed if you verbally agree to it.

Continue to check-in with your assessor every 30 days to ensure you stay on the priority list to be matched to housing. Provide them with any updates to your contact information or circumstances.

If you believe you have been discriminated against or treated unfairly by the TPCH Coordinated Entry System, or if you are a service provider and believe that TPCH has violated its Coordinated Entry policies and procedures, click here to complete and submit a grievance.
